School enrollment underway
By Meet the Teacher, Aug 14-15

School is less than a month away from beginning and it is time to start completing enrollment packets. The link to the student enrollment packet form can be found online at Checotah.kl2.ok.us. Every returning student must have a completed and signed packet. Parents may complete the packet at the link and then the school will print the forms to be picked up and signed on during Meet the Teacher days in the Myrtle Wallace Frost Event Center Lobby.

Meet the Teacher dates are: Monday, August 14 from 2 p.m. – 6 p.m. Tuesday, August 15 from 3 p.m. – 7 p.m.

If you are a new student to the district, your parent may enroll you on these dates: Wednesday, August 2 from 9 a.m. – 2 p.m.

Thursday, August 3 from 9 a.m. to 2 p.m.

The following documents are required for each student: Birth Certificate Shot Records Indian Card (if applicable) Proof of Guardian-ship (if not parents on birth certificate) Proof of Residency Copy of Social Security card.
